With nearly five decades of achievement in a specialty, Dr. Cook serves as a Neurology Consultant with Arizona CRS, in Phoenix and in Yuma, Arizona. He is a respected doctor in his field and noted for his specialty in child neurology.
His vast experience and expertise can be seen in his work history. Dr. Cook served as assistant professor with the department of pediatrics at the University of Texas Southwestern Children's Health in 1976. Prior to that position, he worked as a clinical associate with the medical neurology branch of the National Institute of Neurological and Communicative Disorders at the National Institute of Health; and as medical investigator. He also served as the director of child neurology with the Texas Scottish Rite Hospital for Children.
Prior to his retirement in 2005, Dr. Cook was recognized as the interim director of child neurology with the University of Texas Southwestern Medical Center, where he also served as a clinical professor of neurology.

Dr. Cook’s medical journey began with a Bachelor of Science Degree from Purdue University in 1965. He then attended Duke University where he received his MD in 1968. He went on to complete an internship in pediatrics, a residency in pediatrics, a residency in neurology and a fellowship in pediatric neurology from the Duke University Health System.
He received laboratory training in immunology at the National Cancer Institute and completed coursework in immunology at the National Institutes of Health. He is also a Diplomate of the American Board of Psychiatry and Neurology and American Board of Pediatrics.
To stay abreast of the latest developments in his field, Dr. Cook maintains connections with the American Medical Association, Neurological Rehabilitation Society, Professors of Child Neurology, American Academy of Neurology and Child Neurology Society. He has been involved in a number of civic engagements, including the Dallas Intertribal Medical Committee, the International Foundation for Alternating Hemiplegia of Childhood, the Pharmacy Therapeutics Committee and the University of Texas Medical Branch at Galveston. Dr. Cook was a physician review for National Medical Reviews from 2000 to 2001 and has contributed to numerous articles and medical journals.
A true expert in his field, Dr. Cook has been recognized for several discoveries in his field. Namely, as the first to report on the use of gammaglobulin for chronic relapsing polyneuropathy. He has been awarded Top Neurologist 2017, Top Docs in Phoenix, Arizona by U.S. News Report and today serving as a Patient Preferred Neurological Consultant 2019.
